How to Apply for Joint-Enrollment in Fayette County

Step 1. Complete the SAT/ACT Exam

Students must have SAT or ACT test scores to be enrolled. It generally takes about 3 - 4 weeks to receive scores after taking the exam. However, students may take the exam at Clayton State and have the scores in 1 or 2 days. The on-campus SAT or ACT can only be used to satisfy admission to Clayton State University as a timely way to get into the Joint Enrollment Program. However, this can be a good practice test to prepare for taking a national SAT/ACT exam.  Each test costs ≈$50.00.

If necessary/helpful, an on-campus SAT and/or ACT (test) can be scheduled and taken at the Clayton State University Testing Center (678.466.4495).

Step 2. Apply to Clayton State University

Complete the CSU Application for Admission by downloading the form at the following link. Please use this paper application so that we can identify you as a Fayette County joint enrollment student (there is a fee if you choose to apply electronically):

http://adminservices.clayton.edu/admissions/forms/CSUApp%202006.doc

Note – referencing the CSU Application for Admission (form), don’t forget to include SSN, annotate the Term entering (i.e., Spring 2008), and check (mark) the Freshman Scholar (i.e., JE) block on "entering status" (located at the top of the second page).  Also, the Joint Enrollment student candidate will need to sign this form (not the parent/guardian).  Finally, don’t forget to select a Major, although the annotation of a Major plays no practical role.  If unsure of a major, select the Integrative Studies (B.S. or B.A.) major.

If the completed paper application is returned to the Fayette Office, then the application fee will be waived.  Joint Enrollment student candidates can elect to complete the online CSU application form, if desirable, but the application fee must be paid (online) by the student—for the application to be processed. The online CSU application for admission fee is not reimbursable.

Step 3: Submit Accel Application online and Supporting Documents to the University

The following items should be submitted to Clayton State at the above address.

A. The Accel Application

To complete Part I of the Accel Online Application go to:

 http://www.gacollege411.org/Home/_default.aspx

1.   On the right side of the page below “SHORTCUTS”, select the “>ACCEL” Program hyperlink.

2.   Please read the Accel Program eligibility requirements thoroughly to determine if the student is eligible before completing the online application.

3.   At the bottom of the page, under “Application Procedure”, select the “Online Accel Program Application” hyperlink.

4.   In the Accel Application Form enter the student’s social security number, birth date, and last name. Then select the “Log In” (blue) button.

5.   Select the “Add New Application” hyperlink if you are creating a new or continuing account. (Select the “View Application Status” if you are reviewing your application for eligibility status).

6.   You are now on the “Accel Application – Student Section” page. Enter the student’s:

First name, permanent address, zip code, home telephone number (or best contact telephone number), email address (or best contact email address), gender (If you are a male 18-years-old or older you must provide your selective service ID number, for Selective Service ID information go to: https://www.sss.gov/RegVer/wfRegistration.aspx), state of legal residence, date you became a legal resident, U.S. citizen option button, felony option button, defaulted refund on any federal or state educational loan or grant program option button, select the high school you are currently attending, select the college(s) or universities you have applied to for the Accel Program, and select the, Certification Authorization and Agreement checkbox.

7.   Select the “Submit” button.

8.   If online application is completed correctly the following message will appear:

Thank you  “YOUR NAME”  for using the electronic ACCEL application process. You do not need to complete the paper application nor print and mail your Accel application to the institution you plan to attend. Your Accel application will automatically be available to your high school counselor. If you have not already done so, please schedule a meeting with your high school counselor to review and complete the course information section of the Accel application.

NOTE – when completing the form, the SSN is required.  Also, pay attention to questions pertaining to citizenship status, if not a U.S. Citizen.  In the event the student is an eligible non-citizen, then a copy (front and back) of the Alien Registration card (or other acceptable documentation) will be required.

 B. High School Transcripts

Obtain official H.S. transcripts (in a sealed envelope), hopefully w/SAT or ACT scores annotated.  If the SAT and/or ACT scores are not annotated on the H.S. transcripts, then have the appropriate testing center forward the official test scores to Clayton State.  Pay extra for a rush order, if needed.

C. Immunization Form

Have an authorized Healthcare provider (typically a Physician or Nurse) complete the university Immunization form:

http://adminservices.clayton.edu/admissions/forms/CSU%20Immunization.pdf

Note – copies of immunization records from other sources won’t suffice.  Also, the CSU Nurse Managed Clinic can complete this form and provide needed immunizations.  The clinic has the capability to look at shot records, online.

Note – if unable to acquire all annotated documents (above, A thru C) in a timely manner, then make it a top priority to initially forward the CSU Application for Admission and the GSFC Part I of the Accel application (i.e., first 3 pages).  Receipt of these two documents will get the ball rolling and establish a record of the student, within in the university Banner system.

Step 5. Register for Classes

Once admitted to the university, contact Dr. Mannie Hall at manniehall@clayton.edu to schedule an appointment for registering for your classes. Those wishing to enroll in a math course will need to take a placement test at the main campus.
